What Could Be Feeding Your Pain?

When muscles are tight and painful, it is easy to focus only on where it hurts.

Hi there,

What if your chronic pain keeps coming back because more than one thing is keeping it going?

In this week’s episode, Dr. Jacob Teitelbaum explains four overlapping drivers of chronic pain: muscle pain, inflammation, nerve pain, and what he describes as brain inflammation. He shares why addressing only one may leave other parts of the problem untouched.

Tight Muscles May Need More Than a Massage

When muscles are tight and painful, it is easy to focus only on where it hurts.

Dr. Teitelbaum describes how low energy can leave muscles shortened and tense. His SHINE approach focuses on sleep, hormones, infections, nutrition, and exercise as able—not simply pushing through exhaustion.

He explains why restorative sleep and nutritional support are central to his approach, while cautioning against exercise that triggers a crash.

The conversation also explores how diet may contribute to inflammation and why Dr. Teitelbaum includes nutritional support in his approach to pain. He discusses dietary changes, omega-3s, curcumin, and Boswellia as options he has used with patients.

For nerve pain, he turns to another possible contributor: vitamin B12 deficiency. He explains why he believes it deserves closer attention in people with diabetes, particularly those taking metformin. The discussion highlights why looking at nutritional deficiencies can be part of investigating persistent pain.

The Pain Cycle Can Keep Going

Dr. Teitelbaum compares chronic pain to a fire that can become self-sustaining. In his explanation, brain inflammation can become another part of the problem—not just a response to the original source of pain.

He discusses PEA and low-dose naltrexone as treatments he has used for this component of chronic pain. He also explains why prescription medications can have a place alongside other approaches.

The episode closes with a discussion of stress, the mind-body connection, and the frustration of being told to simply live with pain. Both speakers encourage listeners not to assume that meaningful improvement is impossible.
