Fatigue that sleep does not fix
It usually traces back to one place: the tiny power plants inside every cell you own.
There is a specific kind of tired that confuses people. You slept seven or eight hours. You did not do anything unusual yesterday. And you wake up feeling like you never went to bed at all.
When patients describe that to me, I stop asking about their sleep and start asking about their mitochondria.
The Power Plants Inside You
Every cell in your body contains mitochondria, small structures whose entire job is to convert what you eat and breathe into a molecule called ATP. ATP is the currency your body actually spends. Every heartbeat, every thought, every muscle contraction is paid for in ATP.
You have trillions of these. Your heart muscle cells are packed with them. So are your brain cells and your liver cells, which is exactly why fatigue, brain fog, and poor detoxification tend to travel together.
Here is the number that changes how people think about this: a healthy adult produces and burns through roughly their own body weight in ATP every single day. You are not storing energy. You are manufacturing it continuously, and if the factory slows down, everything downstream slows with it.

What Damages Them
Mitochondria are fragile in a particular way. They generate energy through a process that produces free radicals as exhaust, which means the organelle making your energy is also constantly being damaged by its own byproducts. Normally your antioxidant defenses keep up.
Several things overwhelm that balance.
Chronic infection and chronic inflammation are the big ones. When the immune system stays activated for years, mitochondrial function drops. This is why so many people trace their fatigue back to an illness they never fully recovered from.
Toxic exposure matters enormously. Mold toxins, heavy metals, and pesticides all interfere directly with the energy production chain. So do several common medications, statins in particular, which lower the body's supply of CoQ10, a nutrient the mitochondria cannot function without.
And then there is simple nutrient depletion. Magnesium, B vitamins, CoQ10, carnitine, and alpha-lipoic acid are all required at specific steps. Run short on any one of them and production falls, no matter how much you sleep.
Why This Explains So Much at Once
The reason I keep coming back to mitochondria is that it accounts for the strange combination of symptoms my patients arrive with.
Muscles ache because they cannot generate enough ATP to contract and relax properly. Thinking is slow because brain cells are energy-hungry and get starved first. Exercise makes everything worse for days because you spent energy you did not have. Digestion is sluggish because the gut lining turns over rapidly and needs constant fuel to do it.
One failing system, many symptoms. That is usually the shape of the truth in chronic illness.
What This Means for You
CoQ10 is the place most people should start. Especially if you take a statin. It sits at a central point in the energy chain and depletion is common.
Magnesium runs more than 300 enzyme reactions, and ATP production is one of them. Most Americans are short. Glycinate or malate are absorbed better than the oxide form sold in most pharmacies.
Do not exercise your way out of this. Push a depleted system and you get a crash that lasts days. Gentle movement, short and frequent, until production improves.
Hunt for the ongoing drain. Mold in the house, an untreated infection, a medication, a chronically inflamed gut. Supplements help, but not while something keeps knocking the factory offline.
Give it three months. Rebuilding mitochondrial capacity is slow. Patients who quit at four weeks miss the part where it starts working.
The Bottom Line
Fatigue that survives a full night of sleep is not a sleep problem. It is a manufacturing problem. Feed the machinery, remove what is poisoning it, and stop asking it to do more than it can currently afford.
